Terms and conditions:

1. Please allow 4-6 weeks for delivery of your FREE item.
2. The Epiphone instrument must be NEW.  Previously owned or used instrument do NOT qualify for offer.  Seconds, refurbished or #2 instruments do NOT qualify for offer.#
3. Proof of purchase must be postmarked no later than 15 days after purchase date.
4. Must be 18 years or older.  By returning this form you certify that you are 18 years or older.  (If you are under 18 years old, you may have a parent complete and return this form.)
5. Not applicable with any other Epiphone offer.  Void where prohibited or restricted by law.  Any taxes are your sole responsibility.  This offer may not be sold or traded.  Limit one offer per individual; submissions by groups or organizations will not be honored.  Fraudulent submissions may result in federal prosecution under U.S. Federal Mail Fraud Statutes (18 USC Sections 1341 and 1342).
